Dean’s Directive No. 1/2015
Formal requirements of the Thesis (Bachelor’s, Master’s, Rigorosum, Doctoral)
Art. 1
Under Art. 19 of the Organization of Studies at the Faculty of Mathematics and Physics of Charles University, Art. 8 of the Dean’s Directive No. 7/2014, Art. 4 of the Rigorosum Examination Code of the Faculty of Mathematics and Physics and in accordance with Art. 7 of the Dean’s Directive No. 8/2010 I accordingly stipulate that the form of a thesis should take is as follows:
The thesis is to be written on white paper of A4 format and bound in hardcover in such a way that does not allow replacement of single sheets of the thesis. It must contain the following parts, in the given order:
a) Cover
On the cover must be the
full title of the University and Faculty, indication of the type of the thesis
(Bachelor’s, Master’s, etc.), name and surname of the author, year of
submission of the thesis, and optionally the thesis title. All data must be in
the language of the thesis.
b) Front pageThe
front page must be in the language of the thesis. The requirements are as
follows, in the given order: the full title of the University and Faculty, type
of thesis (Bachelor’s, etc.), Faculty emblem, name and surname of the author,
title of the thesis, name of the department or institution where the thesis was
officially assigned, supervisor (with academic titles), study programme and
study branch, residence of the Faculty (i.e. Prague, in the language of the
thesis), year of submission.

The front page must be in the language of the thesis. The
requirements are as follows, in the given order: Faculty emblem, type of thesis
(Bachelor's, etc.), name and surname of the author, title of the thesis, name of
the department or institution where the thesis was officially assigned,
supervisor (with academic titles), study programme and study branch, residence
of the Faculty (i.e. Prague, in the language of the thesis), year of
submission.
c) Assignment
On a separate sheet there
must be a copy of the signed official assignment of the thesis (only for the
hardback version, not the electronic one). This does not apply to Doctoral
theses.
d) Author’s declaration
On a separate sheet
there must be a signed declaration stating that the student has written the
thesis himself. The declaration is in Czech for a thesis in Czech or Slovak and
in English for other languages:
Prohlašuji, že jsem tuto bakalářskou (resp. diplomovou, rigorózní, dizertační) práci vypracoval(a) samostatně a výhradně s použitím citovaných pramenů, literatury a dalších odborných zdrojů. Beru na vědomí, že se na moji práci vztahují práva a povinnosti vyplývající ze zákona č. 121/2000 Sb., autorského zákona v platném znění, zejména skutečnost, že Univerzita Karlova v Praze má právo na uzavření licenční smlouvy o užití této práce jako školního díla podle § 60 odst. 1 autorského zákona.

I understand that my work relates to the rights and obligations under Act No. 121/2000 Sb., the Copyright Act, as amended, in particular the fact that Charles University in Prague has the right to conclude a license agreement on the use of this work as a school work pursuant to Section 60 subsection 1 of the Copyright Act.

e)
Information sheet
In the language of the thesis and in English the
following information is to be provided: thesis title , author, full title of
the department or institution where the thesis was assigned, supervisor (with
academic titles) and his department, abstract, key words.
f) The
text of the thesis itself
Included in the body of the thesis must be a table of contents, bibliography, list of references, and optionally a list of
abbreviations, tables, pictures, supplements, index. The pages of this part of
the thesis must be numbered.
This directive shall become effective on the date of its issuance.
